6 The following shall be attached to the application: the decision of the relevant political party to nominate candidates for deputy; a protocol of the meeting of the relevant body of political party on nomination of candidates for deputy of local Kengashes, in which surname, name, patronymic, date of birth, profession, position (type of activity), place of work and residence, party membership as well as the name and number of the election constituency shall be indicated; an application by a candidate for deputy of local Kengashes to consent to his/her candidate from the respective election constituency; an application for dismissal from office of the persons indicated in part three of article 90 of the present Code, in case of their election as deputies of local Kengashes.
