| 4 | The application shall include: the decision of the supreme body of the political party to nominate a candidate for the President of the Republic of Uzbekistan; a protocol of the meeting of the supreme body of the political party on the nomination of a candidate for the President of the Republic of Uzbekistan, in which surname, name, patronymic, date of birth, profession, position (type of activity), place of work and residence of the candidate to the President of the Republic of Uzbekistan shall be indicated; an application by the candidate for the President of the Republic of Uzbekistan on his/her consent for nominating his/her candidate; signature sheets supporting the nominated candidate for President of the Republic of Uzbekistan.
